Spartan Dance Center

I, the adult applicant or I, the parent or legal guardian of the applicant listed below, hereby give approval of the applicant’s participation in any and all Spartan Dance Center programs and activities. *

I do waive, release, absolve, indemnify and agree to hold harmless the organizers, sponsors, supervisors, participants and persons involved in the operation of Spartan Dance Center programs for any claims arising out of injury or other loss to named applicant or any member of his/her family whether as a participant in the activities or as a spectator.*

I also give permission for Spartan Dance Center to take photos of me or my child to use for the website and for purposes of promoting the studio.*

I understand that by attending Spartan Dance Center auditions, I am agreeing to abide by the policies and procedures outlined above under team policies.*

I understand that there is a $100 Commitment Fee due with the submission of this application on the date of auditions. This fee covers my dancer's class placements and registration into their fall classes.

• Dancers who are not ready for the competitive program will be refunded the full $100 commitment deposit.

I authorize AAR Ventures (Spartan Dance Center) to charge my card in the above amount for my commitment deposit to the competitive dance team.*
